up for a Kaiser Permanente medical plan today, you have a choice. Kaiser dental can be added now,
or if you prefer you can add it to your medical plan starting in December for a January 1st effective
date of the following year. This is Kaiser Permanente, California's open enrollment period for Delta
Dental on individual and family plans.

There are "stand alone" dental plans from other carriers that can be added at any time, either with or
without Kaiser Permanente medical plans and we would be happy to discuss your coverage options.

coverage (COBRA Benefits) available to you, It is important to know all your options, please check

Blue Shield of California EFFECTIVE DATE: With most applications, medical records are requested and then reviewed. This process takes on average three weeks. Some applications, however go through in only a few days, depending upon health history. The Blue Shield of CA effective date for PPO plans could be any day of the month. The effective date for Blue Shield HMO plans is the 1st of the month. You will be able to request an effective date on your application.
